Applying for Services for your Loved One

If you are interested in knowing if your loved one is appropriate for the C.E.R.T.S. Adult Action Centers, please complete and return the preliminary interest form to Melissa MacCord, Program Manager. This can be accomplished by mailing it to her attention at 52 Reads Way, New Castle, DE 19720 or as a scanned document by email at mmaccord@certsinc.org. You will then be contacted to have your prospective participant come and tour one of our two locations.

When you are serious about applying for enrollment, you need to work with your loved one’s DDDS case manager (family support person) to apply to C.E.R.T.S. Private pay is not accepted.

All referrals for day services must be requested by the family/guardian to the DDDS case manager or family support specialist (CM/FSS).

PLEASE NOTE: DDDS has instructed the CM/FSS not to accept these requests unless the individual is within a few months of needing day services.

Once the request is accepted, the CM/FSS will compile a packet of information about your loved one (including the ICAP, IEP/ELP and other documents) and forward it to C.E.R.T.S.

Once received, our admissions committee will review the paperwork, visit the prospective participant (if we have not already met him/her) to ensure a good program fit and respond back to you and to DDDS within a reasonable time period (typically within 10 business days) as to whether we can support your loved one.

If we can support him/her, we will be in touch to set up a time to have additional documents signed, get up to date information on medicines taken, etc. If we are unable to support him/her, we will provide the reasons why s/he is not a good program fit.
